How to Manage Creatinine 2.1 and Hemoglobin 82 for CKD

I want to know what is the best way to treat the condition of my mother who is diagnosed with CKD secondary to Diabetic Nephropathy. Her creatinine is 2.1mg/dl and hemoglobin is 82 mg/dl.

In clinic, creatinine is used to reflect the level of kidney function, while hemoglobin can reveal the condition of blood circulation. Normally, creatinine level is 0.5-1.1mg/dl for female and 0.6-1.2mg/dl for male, while the hemoglobin level ranges between 120 and 160g/l for male and 110-150g/l for female.
