End Stage Renal Disease

ESRD basics

Definition

End Stage Renal Disease (ESRD) refers to a very serious medical condition featured with the complete or almost complete failure of the kidneys to work. As a result, dangerous levels of toxins build up in body, leading to a myriad of symptoms.

Causes

End Stage Renal Disease (ESRD) usually follows Chronic Kidney Disease. A person may have gradual kidney function loss for 10~20 years or more before progressing into End Stage Renal Disease( ESRD). The most common causes of End Stage Renal Disease (ESRD) include Diabetes and high blood pressure.

Also, some conditions can increase the risk of developing the severe condition including injury or trauma to the kidneys and major blood loss.

Symptoms

※Fatigue and weakness

※Skin problems, such as itchy skin and dry skin

※Weight loss

※Loss of appetite

※Nausea and vomiting

※Headache

※Breath difficulty and breath odor

※Swelling in every area of body

※Concentrating or thinking problems

※Muscle twitching or cramps

Prognosis

In ESRD, as dangerous levels of toxins and wastes accumulate in body and the multiple organs and systems are involved. Without dialysis or a kidney transplant or other intervention treatment, death will occur from the buildup of fluids and waste products in the body.

Prevention

As End Stage Renal Disease (ESRD) is secondary to Chronic Kidney Disease, the the best way to prevent ESRD is to prevent CKD. Diabetes and high blood pressure are the two leading causes of CKD. Therefore, you can reduce the risk of developing ESRD greatly by keeping these conditions in control. Get your blood sugar and blood pressure checked often.

Also, the following habits can help to protect your kidneys, as follows:

*Avoid smoking and drink alcohol in moderation

* Exercise regularly

* Develop healthy dietary habits, eating a low-fat,low-salt diet

* Keep regular-checks

ESRD symptoms

End Stage Renal Disease (ESRD) is a serious medical condition, in which the kidneys fail to work completely or almost completely. As the diseased kidneys fail to remove waste products and excess fluid from the body, it can cause damage to multiple organs and systems through the whole body. You may experience the following symptoms:

Fatigue

As high levels of wastes build up in body, it is very likely to cause fatigue. Also, anemia is a major cause of fatigue. As red blood cells carry oxygen to different parts of body, reduced oxygen supply to body tissues and organs can cause fatigue.

Skin problem

ESRD has been associated with many distinctive skin problems. The skin is often pale which is related to secondary chronic anemia. Extensive bruising of the skin can occur in ESRD because of the problems with platelets and other hemostatic abnormalities. You may have dry skin with a poor texture due to chronic the dehydration. Due to high levels of uremic toxins building in body, you may have a uremia forest with crystallized material found on your face and neck.

Swelling throughout the body

As the kidneys fail to remove fluid from body, it can build up in every part of body like ankle, face, etc. As the lung is usually filled with fluid, you may have difficult breath and even can not lie on back at night.

Nausea and vomiting

ESRD is featured with serious digestive system disorder. It can affect your appetite significantly and you may experience nausea and vomiting. Additionally, the uremic toxins may injure the intestinal tract, thus leading to intestinal bleeding and ulcer.

Nerve system disease

Never system disease is commonly seen in ESRD. It is due to dangerous levels of uremic toxins in body to cause damage to nerve system. In serious case, brain disease may occur and it is a life-threatening condition.

Dialysis or other renal replacement therapies can relieve the above symptoms, but they are not enough to control the disease at all. If a timely and effective treatment is adopted to improve renal function, you will be able to control these symptoms and ESRD fundamentally.

ESRD treatment

In End Stage Renal Disease (ESRD), your kidneys stop working completely or almost completely to filter wastes from blood. Dialysis and kidney transplant are two important treatment options for ESRD. In recent years, with rapid medical technology development, you have more treatment options.

Dialysis

Dialysis is known as artificial kidney. It can take over a part of renal function to filter micromolecular wastes from your blood. However, the residual toxins and wastes can do damage to kidney further. That is why the patients' urine output still keeps reducing while they on dialysis. In a word, dialysis only can clear some wastes from kidneys, but unable to improve renal function at all.

Kidney transplant

Kidney transplant can recover the patients with ESRD normal renal function and extend the patients' life span and improve their life quality. It is a better option than dialysis. However, it is really a tough thing to get a compatible kidney donor. Also, some potential problems and risks of kidney transplant can not be preventable at all. A major challenge of kidney transplant is lifetime immunosuppressive drugs. Long-term use of immunusuppressive drugs can increase the risk of developing a myriad of diseases, such as cancer, Diabetes, etc. Additionally, the reoccurence of Kidney Disease is very high in some kidney diseases, particularly autoimmune kidney disease.

Immunotherapy

Immunotherapy is a new treatment option for ESRD. It is not a single treatment method, but a complete treatment system including removing the wastes from body, eliminating the underlying cause of your Kidney Disease as well as improving renal function by rebuilding kidney structure. It is an integrated therapy of Chinese medicine and western medicines.

Blood purification

Blood purification refers to filter toxins and wastes from body by using a series of blood purification techniques and machines. It mainly include hemodialysis, hemofiltration, hemoperfusion, plasm exchange, immune adsorption, etc. The combined application of these blood purification techniques and machines can remove the varing molecular substances, immune complexes. It can reduce burden on kidneys and create a favorable environment for rebuilding kidney structure and restoring renal function.

Micro-Chinese Medicine Osmotherapy

Micro-Chinese Medicine Osmotherapy is an external therapy and it develops based on traditional Chinese medicine. The effective medicine ingredients can be penetrated into kidney lesions directly through skin and meridians in kidney area. Therefore, it get rids of the defects of traditional Chinese medicines completely.

The above are the major treatment options for ESRD. Apart from the above treatment options, massage, acupuncture, traditional herbal medicines also can help.

ESRD diagnosis

End Stage Renal Disease( ESRD) occurs when the kidneys fail to work adequately to maintain day-to-day life. It almost always comes for Chronic Kidney Disease(CKD) and it is also known as the fifth stage of CKD. The following tests and procedures can help you get a confirmed diagnosis.

1.Medical history

Your doctor will ask about your medical history and symptoms. Also, a physical exam is performed to help determine the correct diagnosis.

2.Blood test

Blood tests are commonly ordered. It includes complete blood count, calcium, phosphorus, and parathyroid hormone, and potassium level.

3.Glomerular filtration rate( GFR)

GFR is used to measure how your kidneys are working to filter wastes and toxins from your body. If your kidneys are damaged severely, GFR will decline significantly. Your doctor can calculate the GFR based on gender, age, body size, and blood creatinine level. Chronic Renal Disease is divided into five stages based on the levels of GFR.

Stage of Chronic Renal Disease GFR
1 over 90 mL/min (normal)
2 60-89 mL/min (mild decrease)
3 30-59 mL/min (moderate decrease)
4 15-29 mL/min (severe decrease)
5 under 15 mL/min (ESRD)

4.Blood urea nitrogen(BUN) and creatinine

It is a measurement to see whether your kidneys are working properly. Creatinine is the metabolic products of muscle and urea is a waste product of protein metabolism. If the kidneys are not filtering blood properly, these metabolic products will build up in blood, leading to increased levels of creatinine and BUN.

5.Blood electrolytes

Kidneys can balance the levels of electrolytes like sodium, calcium, chloride, and potassium and phosphorus. However, in ESRD, the levels of the electrolytes will be abnormal. Potassium, in particular, can quickly rise to dangerous levels.

6.Urine tests

Urine tests are performed to see if your kidneys are working properly. If the kidneys are not working properly, there will be protein loss in your urine. This test will also show whether your kidneys are clearing creatinine at a normal rate. Also, if your kidneys are damaged severely, the urine output will decline significantly.

7. Renal imaging

MRI or CT scan are ordered by your doctor to observe the size and structure of your kidneys. In ESRD, the size of your kidneys will diminish severely except cystic kidney disease like Polycystic Kidney Disease(PKD). Also, the pictures will show the whether urine flow is blocked and whether there is blood flowing through kidneys.

8. Renal biopsy

A piece of kidney tissue is removed for check. It helps determine the cause of Kidney Failure.

ESRD knowledge

End-stage renal disease (ESRD) is the complete, or almost complete failure of the kidneys to function. Here are some of the most concerned aspects about this disease that we're asked most often.

Marriage

End-stage kidney disease is not a simplex disease, but series of common clinical syndrome of advanced stages of kidney diseases. It is the final phase of progressive chronic kidney failure. ESRD is not a kind of hereditary disease and is not infectious, so patients can get marriage. Generally, nephrologists suggested patients to considering marriage when they have a good recovery. Then, they can live a normal life and have a happy family.

Sexual Life

The symptoms of ESRD are very complex, which may occur in every internal organ and tissue of the body. In addition to imbalance of fluid, electrolytes, and acid-base, anemia, bleeding tendency and high blood pressure, patients may suffer from clinical manifestations involved organ system dysfunction and metabolic disorder. So, ESRD patients are not suggested to have sex if they have the above severe symptoms.

Sexual life have certain effect on the kidney disease. However, patients can have regular and moderate sexual activity according to their own disease condition. Meanwhile, they need to adopt effective treatment to protect the residual kidney function and avoid further deterioration. Over time, the renal functions can be restored with holistic treatment.

Pregnancy

Women with ESRD are not advised to get pregnant. There may be large amounts accumulation of toxins within the body, which affects the normal levels of hormone, leading to reduced ability to fertile. Even if they get pregnant, there is great risk of abortion and premature delivery though pregnancy is sometimes successful.

Therefore, we recommend that ESRD patients should try to treat their illness effectively. After the condition becomes better or stable, they can plan to have a baby. This is beneficial to both the mother and the baby.

ESRD healthcare

End Stage Renal Disease (ESRD) occurs when your kidneys stop to function well enough to filter blood and discharge wastes and metabolic products from body. While receiving medical treatment, you can follow a good health care, this contributes to a better recovery.

▪ Diet.

Control fluid intake. Limit the intake of foods rich in fluids such as melons, celery, oranges and tomatoes. In addition, sodium can cause the body to retain the fluids and aggravate swelling and high blood pressure. For this reason, low-sodium foods should be adopted.

Restrict high- potassium and phosphorus foods. In ESRD, your kidneys fail to keep the electrolyte balance. As a result, minerals will be out of balance. If potassium builds up in the blood, it can cause irregular heart rhythm which can be dangerous. You should limit the fruits high in potassium such as banana, orange, avocados, kiwis, and dried fruits. However, if you are on dialysis, you may loss potassium in the process. So you should not have so tight control on potassium intake. Phosphorus causes the bones to lose calcium, which can lead to osteoporosis. Additionally, high blood phosphorus can cause skin itching. Dairy products, nuts and peanuts and other foods high in phosphorus should be eaten sparingly.

Limit intake of foods high in protein, because proteinuria can aggravate kidney deterioration. However, if you are on dialysis, you should increase the intake of high-quality protein foods such as fish, poultry, and egg whites.

▪ Daily Life.

Keep the ward with good ventilation, and proper temperature.

Have a good control of blood pressure, but the level should not be too low, which may lead to insufficient blood supply of tissues and organs of the whole body.

Guarantee to have enough rest. Overwork can reduce the resistance of human body and induce infection, thus exacerbating the kidney damage. So appropriate rest is beneficial to protect the kidneys and recover from kidney disease.

Prevent catching a cold or flu. Cold can lead to the relapse of kidney disease, and rapidly deterioration of illness condition.

▪ Mental Health

Doctors and family members should encourage ESRD patients to establish the confidence to overcome the kidney disease with comprehensive treatment and health care. Have a correct attitude toward the renal disease. Good mental state plays an important role in successfully defeating the disorder.
