How Dose Renal Failure Cause Metabolic Acidosis

2013-08-27 13:50

Metabolic acidosis is commonly seen in patients with renal failure, particularly once glomerular filtration rate (GFR) drops below 25ml/min.Knowing the causes of metabolic acidosis can help the patients manage it better.

Causes of metabolic acidosis in renal failure

The body’s cells use chemical reactions to carry out tasks such as turning food into energy and repairing tissue. These chemical reactions generate acids. In right condition, kidneys can remove extra acids to maintain acid-base balance in body.

However, in Renal Failure, the kidneys fail to work normally. As a result, high levels of acid will build up in body, resulting in metabolic acidosis. Too much acid-acidosis-ca disturb many bodily functions, resulting in a host of adverse effects on body.

Adverse effects of metabolic acidosis in renal failure

Development or exacerbation of bone disease

Growth retardation in children

Reduction in albumin synthesis

Increased degradation of muscle with muscle wasting

Increased risk of developing cardiovascular disease and mortality

Impairment of glucose tolerance

How to treat metabolic acidosis in renal failure?

Since metabolic acidosis has so many adverse effects, the patients are eager to find an effective way to correct the condition.

Base therapy to ameliorate the acidosis should be conducted by the renal failure patients. In patients who are not on Dialysis, serum [HCO3−] can be increased by administering base in the form of oral bicarbonate or organic anions that are metabolized to bicarbonate.

In the patients with end stage renal disease (ESRD), the patients can use a dialysate containing 39–40 mEq/l of HCO3− to correct acidosis. If the patients do not respond to high dialysate [HCO3−],the addition of oral base is usually effective.

The above ways only can relieve acidosis in renal failure, but can not treat the underlying cause. To treat it completely, the treatment should focus on improving the excretory ability of kidneys. For more information about this topic, you can email to kidneyabc@hotmail.com .
