High Blood Pressure in Nephrotic Syndrome

High blood pressure is commonly seen in nephrotic syndrome. It is a significant dangerous factor in increasing protein excretion and worsening renal function.

Healthy kidneys can filter blood, removes extra fluids and wastes from the body, and release hormones that regulate blood pressure. However, if kidneys are not working well, they will lose the functions. High blood pressure in nephrotic syndrome is mainly due to retention of water and sodium in body.

Each kidney is composed of 1 million nephrons and each nephron consists of a filtering unit of blood vessels called a glomerulus, which is attached to a tubule. In right condition, the kidneys can process roughly 150 to 200 quarts of blood each day through nephrons. After blood is filtered by nephrons and it will enter the tubules, where most of the filtered sodium and water is removed. The filtrate that exists the tubule is excreted as urine.

However, in nephrotic syndrome, the glomeruli are inflamed and can not function normally. As a result, a large amount of sodium and water retain in body, resulting in increased blood volume. Eventually, high blood pressure occurs.

To control high blood pressure in nephrotic syndrome, diet plays a vital role. People with the condition should remove high-salt foods from their diet, especially processed foods.

Water pills are usually used to help the secretion of fluids. It is helpful in managing high blood pressure. It is very likely to cause loss of potassium in urine.

ARB inhibitors and ACE inhibitors are usually prescribed to lower high blood pressure. These medications are also effective in controlling proteinuria and protecting renal function for nephrotic syndrome patients.

The above treatments can help lower high blood pressure, but can not control it completely.

As high blood pressure is the result of impaired nephrons, the treatment goal should restore the impaired kidney structure and recover renal function. If the extra fluids and sodium can be removed from body, high blood pressure in nephrotic syndrome will be controlled completely.
